の維持に関わる労務管理の実践を記述することを目的とした。研究デザインは記述的研究である。調査方法はインタビューガイドを用いた半構造的面接法とし、24 時間緊急訪問対応体制をとるST の訪問実績と組織編成を聴き取りと提供資料から把握した。ST が提供する看護内容30 項目の年間総提供回数の約10%は医療的ケア度の高い「特別管理加算」項目だった。管理者は「特別管理加算」は「重い」と語った。その「重さ」とは、ST の収益上無視できない重さという意味合いと、医療依存度の高い利用者の病状の深刻さ、確かな医療的ケアを提供することに対する看護師の緊張感を示すものと分析した。ST を維持するために労務管理で意識していることには「求人に経費をかける」「看護の質を上げる」「スタッフを『島(しま)』に分ける」の3 つの実践が語られた。①求人のために安価とはいえない有料人材紹介業を利用し、人員確保に経費を投入することで採用の確実性を高めていた。一方で財政黒字を維持するST でなければこうした採用の仕方は容易でないと理解されているとわかった。②「質の高い看護」の提供がST の利益につながると理解されていることがわかった。③質の高い看護の提供には職員の教育が欠かせず、教育が伴わない雇用では利用者のニーズに応えることはできないとの理解のもと、OJT の目的で、実践のための細かな情報交換や活動の視点を伝達する場を「島」と呼ばれる5
